  • Adam Air Flight 574 was a scheduled domestic passenger flight operated by Adam Air between the Indonesian cities of Surabaya and Manado that disappeared near Polewali in Sulawesi on 1 January 2007. The plane, a Boeing 737-400, was ultimately determined to have crashed into the ocean. All 102 people on board died, the highest death toll of any aviation accident involving a Boeing 737–400. A full national investigation was immediately launched into the disaster. The final report, released on 25 March 2008, concluded that the pilots lost control of the aircraft after they became preoccupied with troubleshooting the inertial navigation system and inadvertently disconnected the autopilot. The crash is one of several transportation accidents, including the subsequent non-fatal crash of Adam Air Flight 172, which among them have resulted in large-scale transport safety reforms in Indonesia, as well as the United States downgrading its safety rating of Indonesian aviation, and of the entire Indonesian fleet being added to the list of air carriers banned in the EU. Adam Air was subsequently banned from flying by the Indonesian government, and would later declare bankruptcy. Wikipedia
