In this episode there are three separate story arcs that involve J.D. dealing with Kim's pregnancy, Dr. Cox's anger and the Janitor's fear that he is wasting his life. In each arc the character has a patient (in the Janitor's case, simply a new friend) who is portrayed by the actor who plays the respective character, as though they are looking in a mirror.
