Ishii is an American of Chinese and Japanese heritage. When she was still very young, around nine years old, her parents were brutally murdered by a Yakuza crime lord, Boss Matsumoto, before her very eyes, while she hid in terror under her parents' bed. Ishii's father, a martial artist and member of the military, tried to fight off their attackers, but was soon overpowered and stabbed by (unbeknownst to O-Ren) Bill; then, Boss Matsumoto himself dragged Ishii's mother to the bed and stabbed her as well, almost killing young Ishii as the katana's blade went through the mattress, nearly penetrating Ishii's head. After this, Bill burned down the house, and little Ishii barely escaped.
