Smithsburg High School's sports program is best known for its girls teams, having won a total of 34 Maryland state championships in the 1A or 1A/2A division. The boys teams, by contrast, have won only 14 state championships. The school also has boys and girls lacrosse teams, a baseball team, and a boys soccer team though none have as of yet won a state championship.
