On Monday, November 20,1965, Convair 880 N821TW, with 18,846.75 hours of flying time, departed Los Angeles International Airport at 5:37 PM EST with 75 passengers and a crew of 7. The airliner was operating as TWA flight 128 and bound for Boston, Ma. with intermediate stops in Cincinnati, Ohio and Pittsburgh, Pa. Once at cruising altitude, early afternoon in California quickly turned into early evening as the plane traveled eastbound for the Greater Cincinnati Airport.
Light snow was falling in the Cincinnati area as flight 128 received clearance for an instrument landing on runway 18 in Northern Kentucky. The approach was routine although a little rapid. With the airport in sight, flight 128 crossed the Ohio River Valley, only to strike a tree in an apple orchard and impact the ground 9,357 feet short and 429 feet right of the centerline to runway 18.
