J.D.'s narration reveals that, since visiting his brother the month before, Turk has been acting more competitively than usual. Carla casually reveals that Turk lied about his "vacation", and had in fact had a testicle removed after a bout of testicular torsion, caused by a playful kick to the groin from Izzy. J.D. is hurt that Turk had kept this from him, eventually forcing Turk to admit that the surgery had left him insecure about his masculinity.
Meanwhile, The Janitor starts a hospital newsletter, "The Janitorial", which airs the hospital's dirty laundry. Finally, Elliot must a convince a male patient with breast cancer to tell his macho brothers about the disease, while trying to help Dr. Kelso keep his job.
